I hereby agree to and authorize the creative use of my name, title, photograph, and voice in any positive campaign announcement, advertisement, publication, literature, newspaper, radio, television, public announcements, other forms of media outreach, and fundraising activities in both digital and hard copy in support of Madison O’Malley for election as Louisiana State Representative, District 91.
